Is it possible to have more than two input fields?

To answer you question, as far as I can tell you can’t incorporate more than two input fields. And, it’s not because of your email service provider’s form generator but rather because of the way each popup template is designed.
Of course, if you know CSS I’m sure you could edit their CSS file to increase the height of the popup window.

Regarding your question, you’ll need to copy and paste the optin form code your receive from your email marketing software provider like aWeber into PopUp Domination before it will work.
Then, whatever autoresponder emails you have setup through aWeber will automatically be sent when someone enters their information into PopUp Domination’s optin form.
